What this is

Health Appeal provides general information about health insurance appeal rules, and document templates you can use to write your own appeal. It is free.

What this is not

Health Appeal is not a law firm and is not a substitute for a lawyer. It does not give legal, medical or insurance advice. Using it does not create an attorney-client relationship or any professional relationship. It does not evaluate whether you have a legal claim, and it does not refer you to lawyers. It is not affiliated with any insurer or government agency.

You are the author

You decide what your appeal says. You review it, sign it and send it. You are responsible for its accuracy and for meeting your deadline.

Deadlines

We take deadlines from published federal rules and review them, but rules change and plans differ. The deadline and address printed on your own notice always control. If they differ from what Health Appeal shows, follow your notice. When in doubt, send the appeal early.

Your AI assistant

Health Appeal supplies rules and templates to AI assistants made by other companies. We do not control what an assistant writes. Read every draft before you sign it.

No guarantee

Health Appeal is provided as is, without warranties. We cannot promise any result from an appeal. To the extent the law allows, Health Appeal is not liable for losses arising from use of the service.

Fair use

Do not use the service to submit false statements to an insurer or agency, or in a way that disrupts it for others.
